Why Correct Calcium for Albumin?

Approximately 40-45% of total serum calcium is bound to proteins, primarily albumin. The remaining fraction is mostly free (ionized) calcium, which is the physiologically active form.

When a patient has hypoalbuminemia (low albumin levels), the total measured calcium will appear lower because there is less protein to bind to it. However, the free (ionized) calcium may still be normal. Calculating the corrected calcium helps estimate what the total calcium would be if the albumin were normal, preventing misdiagnosis of hypocalcemia.

Corrected Ca = Measured Ca + [0.8 × (Normal Albumin - Patient's Albumin)]

Interpretation (General Guidelines)

  • Normal Range: ~ 8.5 - 10.2 mg/dL
  • Hypocalcemia: < 8.5 mg/dL
  • Hypercalcemia: > 10.2 mg/dL

Limitations

The correction formula is an estimation and has limitations. It may be inaccurate in the following situations:

  • Critically ill patients: Acid-base disturbances significantly alter calcium binding to albumin. Acidosis increases ionized calcium, while alkalosis decreases it.
  • Severe hypoalbuminemia: The formula is less accurate at very low albumin levels (e.g., < 2.0 g/dL).
  • Extremes of age.
  • Massive blood transfusions: Citrate in blood products can bind calcium.

Gold Standard: The most accurate way to assess physiological calcium status is by measuring the ionized calcium directly.

Case Example

Patient: 65-year-old malnourished male. Measured Ca: 8.0 mg/dL (low), Albumin: 2.0 g/dL (low).

Calculation: Corrected Ca = 8.0 + 0.8 * (4.0 - 2.0) = 9.6 mg/dL.

Interpretation: Corrected calcium is 9.6 mg/dL, which is within the normal range. The apparent hypocalcemia was due to low albumin.

Frequently Asked Questions

Why do we correct calcium for albumin?

About 50% of total serum calcium is bound to albumin. If albumin is low, total calcium drops, but the biologically active "ionized" calcium may still be normal. Correcting estimates the true physiological calcium status.

When is ionized calcium better?

Ionized calcium is a direct measurement of active calcium and is preferred in critically ill patients, those with acid-base disturbances, or severe liver/kidney disease, where correction formulas may be less accurate.
